Call for information

We know that multidisciplinary and integrated care in heart failure works.

Multidisciplinary and integrated care is well established in European and national heart failure guidelines, yet implementation of such care varies significantly across Europe. To begin to increase access to these approaches, there is an urgent need to better understand opportunities and barriers that people face in setting up and running these essential programmes.

Help others learn from your experience of implementing multidisciplinary care in heart failure.

We have launched a call for information on 14 February 2018 to find examples of care models, leadership and tools that can be shared across Europe to help other centres plan and deliver multidisciplinary and integrated care for HF patients.

What happens after the call for information?

We will launch a new Handbook on multidisciplinary and integrated care in heart failure, in May 2018 – showcasing case studies and learnings from this call for information.
Ultimately, we hope this call for information and the Handbook will be of value to all those working to improve the lives of heart failure patients.
